When submitting a loan, your lender will require certain documents and information before approval. Here's a helpful list of what will your lender will need to approve your submission.

From You:

1. Personal Finance Statement and Tax Returns (Prior 3 Years)

2. Business Plan for Practice

3. Resume or Curriculum Vitae

4. Copy of Healthcare License and Driver’s License

5. Copy of Life and Disability Insurance Policies

6. Copy of Malpractice Insurance

7. Credit Report


From the Seller:

A. Tax Returns for the Prior 3–5 Years

B. Current Profit & Loss Statement

C. Staff Census (employee names; salaries; benefits, etc.)

D. Copy of Buy/Sell Agreement

E. Allocation of Purchase Price

F. Practice Appraisal
